Transitional Housing

The VA's Homeless Providers Grant and Per Diem Program is offered annually (as funding permits) by the Department of Veterans Affairs Health Care for Homeless Veterans (HCHV) Programs to fund community agencies providing services to homeless Veterans. The purpose is to promote the development and provision of supportive housing and/or supportive services with the goal of helping homeless Veterans achieve residential stability, increase their skill levels and/or income, and obtain greater self-determination.

Program Eligibility:
• Individual Veteran
• Honorable discharge of any branch of the US Armed Services
• Clean and sober for at least 28 days
• Willing to actively search for employment within the first thirty (30) days of residency
• Able to transition to independent living in less than two (2) years

Services Available:
• Clothing
• Case Management
• Food Baskets
• Daily Living Skills (Budgeting, Job Searches, Grooming, Finding Permanent Housing, Driver’s License/Bus Schedules, Referral Services)
• Transportation to VA/Appointments, Shopping, and Events
• Training & Employment
